Cigna Global for Paraguay: An Honest Review
Cigna Global is the plan long-term settlers, retirees, and families tend to land on once they outgrow travel-style cover. It costs more and asks more questions, but it covers what nomad plans will not. This review explains the tiers, the underwriting, and the trade-offs so you can tell whether it is the right fit for a Paraguay move.
Quick Verdict
Good for: long-term settlers, retirees (no age cap), families, and anyone with a pre-existing condition who can pass underwriting. Wrong for: short scouting trips and healthy young nomads on a budget, who should start with SafetyWing (Essential about $62.72 for four weeks) or travel insurance.
Plan Structure
Cigna Global is built modularly. You start with Core International Medical Insurance (inpatient and emergency cover) and add the modules you need.
Core (required)
Inpatient hospital treatment, emergency treatment, and day-case care. The foundation you build on.
Optional modules
- Outpatient (routine consultations, diagnostics)
- Health and Wellbeing (preventive screenings)
- Vision and Dental
- Medical Evacuation and Crisis Assistance
Coverage tiers
| Tier | Annual maximum | Notes |
|---|---|---|
| Silver | $1,000,000 | No maternity |
| Gold | $2,000,000 | Maternity to $7,000 |
| Platinum | Paid in full (no cap) | Highest limits; maternity to $14,000 |
Areas of cover are Worldwide or Worldwide excluding USA. Including the USA raises the premium sharply. If you pick Worldwide excluding USA and need emergency care while visiting the USA, the core plan still covers out-of-area emergency hospitalisation, with sub-limits of $100,000 on Silver, $250,000 on Gold, and paid in full on Platinum.
What the Outpatient module adds
A frequent Paraguay misconception is that the Core plan covers routine doctor visits. It does not. The Core covers inpatient, day-case, and emergency treatment; outpatient consultations, prescribed outpatient drugs, and outpatient rehabilitation require the optional Outpatient module. That module has its own annual maximums: $15,000 on Silver, $35,000 on Gold, and paid in full on Platinum, with specialist consultations capped at $2,500, $7,500, and paid in full respectively. If you expect to use private sanatorios in Asuncion for everyday care, the Outpatient module is usually worth adding.
The Outpatient module also carries a dedicated line item for older beneficiaries: the "60+ Care" benefit for pre-existing-condition care, set at $500 (Silver), $1,000 (Gold), and $2,000 (Platinum) per beneficiary per period of cover. It is not a substitute for full pre-existing cover, but it signals that Cigna builds later-life care into the plan structure.
Medical Evacuation and the Paraguay Corridor
The optional Medical Evacuation and Crisis Assistance Plus module covers reasonable transport costs to the nearest centre of medical excellence when treatment is not available locally in an emergency, plus medical repatriation and a compassionate-visit benefit. This matters more in Paraguay than in many expat destinations, because complex cases that exceed what Asuncion private hospitals can handle are typically transferred to Buenos Aires or Sao Paulo, and a stretcher transport on a commercial flight runs $25,000 to $30,000 plus seats, with air ambulance costing far more. The CDC notes medical evacuation can run from about $25,000 within North America to well over $250,000 for a distant, complex case.
When evacuation is triggered
The insurer, not the member, decides when evacuation is medically necessary, usually after local hospitalisation with an anticipated multi-day stay or treatment not available locally. Keep the Cigna membership card with you so the Asuncion sanatorio can call for a guarantee of payment.
Standalone medevac as an alternative
If you skip the evacuation module, a standalone medevac membership (such as Medjet) can complement Core cover. Unlike insurer evacuation, some memberships transport you to your home hospital rather than only the nearest adequate facility.
The Pre-Existing-Condition Question
This is the single biggest reason readers choose Cigna Global over SafetyWing, which excludes pre-existing conditions outright. Cigna offers two underwriting paths:
Moratorium underwriting
A pre-existing condition is excluded for a 24-month period. If you have no symptoms, treatment, or consultations for it during that window, cover for it may then be included. Less paperwork up front, but you wait for certainty.
Full medical history declaration
You disclose your history up front and get certainty from day one, with possible exclusions. More paperwork, but you know exactly where you stand before you move.
Cigna also offers a dedicated later-life outpatient line for pre-existing-condition care. Note that serious conditions can still be declined or excluded even after full declaration.
What It Costs
Cigna Global publishes no flat price; it is quote-based and scales with age, modules, deductible, and USA inclusion. A representative range is about $200 to over $800 per person per month. Indicative ex-USA figures (no deductible) illustrate the curve:
| Age | Silver (indicative) | Platinum (indicative) |
|---|---|---|
| 33 | ~$202/mo | ~$362/mo |
| 53 | ~$337/mo | ~$604/mo |
| 65 | ~$548/mo | ~$982/mo |
Figures are indicative ex-USA quotes from independent reviews; including USA more than doubles the premium. Get a personal quote for your situation.
Underwriter, Trust, and How Claims Work
Cigna Global policies are underwritten by Cigna Global Insurance Company Limited, based in Guernsey and authorised by the Guernsey Financial Services Commission. The product is built for international mobility, with cover in more than 200 markets, access to a network of over 2 million health professionals, and a 24/7 multilingual service line.
On trust signals, the Cigna Global expat trustpilot score sits around 4.1 to 4.2 out of 5 across thousands of reviews. Treat the score carefully: it is the expat product, distinct from the much more negative US-domestic Cigna reviews. An independent ProPublica investigation into a system called PxDx found Cigna's US-domestic employer plans rejected large numbers of claims very quickly without opening patient files, and Congress followed up. That investigation concerned US-domestic plans, not necessarily the Cigna Global expat product, but it is a brand-level risk worth knowing about.
Claims mechanics in plain terms
- In-network direct billing. For inpatient and day-patient treatment, Cigna aims for a 1-hour guarantee of payment to the hospital after authorisation, and direct-settles where an arrangement exists.
- Out-of-network reimbursement. Where there is no direct-billing arrangement (common in Paraguay), you pay the sanatorio, keep every receipt and discharge summary, then submit a claim. Cigna aims to process claims within 5 working days.
- Prior authorisation. Inpatient and day-case treatment requires prior authorisation, so call Cigna before non-emergency treatment. Emergency admission is covered first and authorised after the fact.
- Minimum cover period. The policy carries a minimum three-month cover period before cancellation, so it is not a month-to-month nomad subscription.
Where Cigna Global Falls Short
- Premium cost. Roughly three to four times SafetyWing for the same healthy young adult, and far more at older ages.
- USA-included pricing. Worldwide cover including the USA more than doubles the premium.
- Underwriting rigor. Serious conditions can be declined or excluded even after a full declaration.
- Plan complexity. Modules, deductibles ($0 to $10,000), cost-share (0 to 30 percent), and out-of-pocket maximums take effort to work through.
- Paraguay direct-settle friction. Cigna's hospital finder is member-login-only, so there is no public list of direct-billing facilities in Paraguay. Reimbursement is therefore the norm; confirm any direct-billing arrangement with the sanatorio and through Cigna Envoy in advance.
- Claims-experience variance. Independent ratings are mixed; treat reviews as anecdotal and specific to your policy and region.
- Sign-up effort. The application and underwriting process is more involved than buying a monthly nomad plan.
- Not travel cover. This is health insurance, with no trip cancellation, baggage, or personal-liability cover.
Who Should and Should Not Buy It
A good fit if you are
- A long-term settler or retiree (no age cap)
- A family wanting broad international cover
- Managing a pre-existing condition and able to pass underwriting
- Wanting higher limits and evacuation options than nomad plans
Look elsewhere if you are
- On a short scouting trip (use travel insurance)
- A healthy young nomad on a budget (use SafetyWing)
- Only needing routine care (cash or a local prepaid plan is cheaper)

Frequently Asked Questions
Is Cigna Global good for Paraguay?
Cigna Global is a strong fit for long-term settlers, retirees, and families in Paraguay, especially anyone with a pre-existing condition who can pass underwriting. It has no upper age limit (unlike SafetyWing, whose Complete plan ends at 64) and offers much higher coverage limits and medical evacuation options. It is overkill for a short scouting trip, where travel insurance or SafetyWing costs far less.
How much does Cigna Global cost per month?
Cigna Global is quote-based and varies widely with age, modules, and whether you include the USA. A representative range is about $200 to over $800 per person per month. To illustrate, indicative ex-USA quotes with no deductible run roughly $202 for a 33-year-old on Silver and about $548 for a 65-year-old on Silver. Including USA coverage more than doubles the premium. Treat any figure as indicative and get a personal quote.
Does Cigna Global cover pre-existing conditions?
It can, which is the main reason people choose it over SafetyWing. Cigna Global offers two underwriting paths. Moratorium underwriting excludes a pre-existing condition for a 24-month period; if you have no symptoms, treatment, or consultations for it during that window, cover may then be included. A full medical history declaration discloses everything up front for certainty from day one, with possible exclusions. Serious conditions can still be declined or excluded.
Does Cigna Global have an age limit?
No. Cigna Global has no upper age limit, which is a key differentiator. SafetyWing Complete ends at age 64. For retirees relocating to Paraguay, this makes Cigna Global one of the few viable international plans at older ages.
Does Cigna Global direct-bill at hospitals in Paraguay?
Cigna Global does not publish a Paraguay direct-billing hospital list; its hospital finder is member-login-only, so there is no public directory to check before you enroll. Coverage in Paraguay therefore runs on a pay-and-claim-reimbursement basis, with direct billing only at select facilities you confirm in advance through Cigna Envoy or with the sanatorio. For emergencies, Cigna typically arranges direct settlement where possible. Keep all documentation for reimbursement claims.
How does Cigna Global compare to SafetyWing for Paraguay?
They serve different readers. SafetyWing Essential runs about $62.72 for four weeks and suits healthy nomads under about 40, but excludes pre-existing conditions and caps medical evacuation at $100,000 lifetime. Cigna Global costs several times more, but offers far higher limits, optional evacuation modules, no age cap, and a path to pre-existing-condition cover. Nomads usually start with SafetyWing; long-term settlers and retirees usually move to Cigna Global.
